The treatment challenge posed by narcissistic personalities is illustrated in the description of the psychoanalytic treatment of a young woman. The complexity of establishing a diagnosis, the full expression of the narcissistic defenses, the components of the negative transference, and the outcome of the treatment are presented and discussed in detail.
