The clinical vignette poses a number of questions which are essential to consider, first in order to make a tentative diagnosis and then to be able to understand what may be happening in the treatment and in the session described.
Firstly, I note that she was put on anti-depressants and anti-psychotic medication at the age of 18 and has remained on them for the next 15 years until the present time. Though we are not told of the presenting problems she showed at the age of 18 we must presume that they were psychotic
