The setting and ground rules in psychoanalytic psychotherapy definene the nature and boundaries of the therapist-patient relationship. This paper scrutinizes the consequences for both parties when a therapist agrees to shift the setting from the office to the home, during the management of an adolescent's crisis. The literature on this matter is divided. Some therapists believe that to offer this facility is potentially disastrous, as it stimulates a patient's omnipotence. Other clinicians feel that, given these circumstances, a flexible frame should prevail. In this case, despite some acting out that occurred during the establishment of a safe home setting, the therapist did manage to develop a stable, interpretative space through the analysis of the transference-counter-transference dynamic, which led to mutative changes in the patient's mind. The patient realized that what he had projected into the therapist was a fragment or a split-off part of him. Upon reintegrating this, he developed greater ego strength and returned to the consulting room to complete his psychotherapeutic journey.
