Objective. there is controversy about whether psychotherapies from different schools rely more on similar “non-specific” interventions or on interventions theoretically consistent with the treatment orientation. This study compares psychodynamic (DYN) and cognitive-behavioural (CPT) therapy sessions with depressed patients.
Methods. Twelve patients with recurrent major depression were admitted into a randomized pilot study of antidepressive medications combined with 20 sessions of either psychodynamic psychotherapy or cognitive-behavioural therapy. Transcripts of an early and a mid-phase session (numbers 3 and 14) were obtained from each patient, save for an early dropout. The remaining 11 patients had 22 sessions rated by independent and blinded raters using the Comprehensive Psychotherapy Intervention Rating Scale (CPIRS).
Results. Both treatments used 48.4% of the interventions in common, especially coaching, directive process, and authoritative support interventions, whereas they differed in the remaining 51.6%, largely along theoretical lines. DYN used more psychodynamic and psychoanalytic interventions with trends toward more facilitating, experiential, and other interventions. Cognitive-behavioural therapy used more cognitive and structuring (trend) interventions. The 11 therapies could be correctly classified by these differences at a highly significant level in both the third (p <.0001) and
fourteenth session (p <.004). The profile of interventions was stable from early to mid-phase sessions in both DYN (.94) and CPT (.83).
Conclusions. DYN and CPT use roughly and equal number of interventions in common, while the theoretically specific types of interventions discriminate them well. Additional studies should examine larger samples, and include psychoanalysis as a distinct treatment.
Conclusions.
