The wishes for freedom must arise from the patient and not be determined by the physician's conception of what the patient should desire. Therapists should avoid patients whom they fear. Closed wards must be considered therapeutic and not punitive. When a psychotic patient refuses to come to the doctor, then the therapist should go to the patient.
