Patient, a man of thirty-six years, married, one child (a boy), came to analysis in order to overcome his homosexuality and his dislike of his wife. He is very mean to her; loves his child as a narcissistic glorification of himself; would have divorced his wife if she had borne him a girl-child (patient lived in Egypt for many years). Patient has had intercourse with women on several occasions, mainly to prove his normality to other people. When in an occasional state of anxiety, patient is glad of the presence of his wife.
Compulsory features: Has to weight himself several times per day. Very happy when he gains weight. Strong dislike of cats which decreases with progress in analysis.
Strong fixation to his eldest sister, running parallel with strong homosexual tendencies and activities; change to indifference to her opinions when homosexuality decreases.
Strong antipathy towards the eldest brother running parallel with strong homosexual tendencies and activities and/or masturbation; change to positive co-operation and friendliness when homosexuality decreases.
Attitude to work: Has been able to avoid discipline and work all his life. This attitude changes with decrease of homosexuality.
Two special features:
1The patient's first conscious and homosexual masturbation phantasy was of playing with the anus and the buttocks of a policeman.
2The patient 'decided' during analysis to give up his homosexual practices (partly in the unconscious hope of avoiding further analysis). During this time of abstinence (lasting about nine days) the patient felt the sudden desire to masturbate and did so. He was very upset about this. It transpired that he felt masturbation to be worse than homosexual practices.
With regard to (1) his attitude to homosexual adventures is that he feels himself as 'top-dog'. In spite of (or because of) a rather feminine approach and great vanity about his looks, he considers that he is taking the male rôle. He would never allow anyone to have int
