That which is repressed in one culture may be overt in another culture and vice versa. This paper reports the dream of a female patient in which occurred all but one of the many phallic mutilations known to anthropologists. The patient's dream and associations is followed by an account of an imaginary ailment encountered in parts of South-East Asia and known as koro. The patient fears that his penis will retract into the abdomen. A Dutch psychiatrist has suggested that the practice of inserting a small rod (ampallang) into a hole pierced through the glans penis is to prevent the retreat of the penis into the abdomen. The author reports that amongst the Mohave Indians jokes are made about male transvestites who would like to push the penis into the abdomen and so produce a vagina. The feminization phantasy which in Indonesia and South China causes anxiety is turned into a joke by the Mohave Indians. The patient's dream, while referring to other genital mutilations, omitted any reference to the ampallang. The author suggests that the patient unconsciously believed that she was already in a state of koro. Her penis had already 'withdrawn' into the vagina. This could never have happened if her phantasied penis had been provided with an ampallang in the first place. The patient knew of circumcision, sub-incision, and castration, but knew nothing of other mutilations such as encrustation which did appear in her dream or of the ampallang which did not appear in the dream. In this case phantasy material which in certain areas is culturally standardized and implemented by means of formal and traditional acts and emotions appeared in a patient's dream as a purely autistic production.
