This is a case presentation the aim of which is to demonstrate the unconscious phantasy which came to be associated with the foreskin. The patient was a boy of 7 who suffered from conversion hysteria. That he was not circumcised in contrast to his father and brother accentuated his feminine identification. He, like his young sister, had no glans penis but only a fold of skin comparable to her 'crack' which was also composed of folds of skin. This led to a bisexual phantasy—he was both boy and girl.
