The author presents a summary of the important papers in the psychoanalytical literature dedicated to the study of the negative therapeutic reaction. These papers are those she used to characterize certain rules and theoretical–practical knowledge that gave direction to this present paper. She then describes a patient with a disturbance of sleep rhythm, linked with a strong narcissistic structure, and considers her as a case of negative therapeutic reaction.
Without rejecting the patient's different aspects, the author desires to emphasize that, in this case, the contact of the patient with his infantile parts, experienced as dependent and weak before his love objects, released a form of negative therapeutic reaction, which was expressed in the form of a sleep disturbance. In work with this type of patient, the author stresses the fundamental importance of the analysis of envy, the defences used against the feeling of envy, and of aggressiveness, as a form of work that showed itself adequate and useful in obtaining the slow and gradual collaboration and integration of the patient.
