To reflect on Ms. A, a case of masochism, Sjögren's syndrome and transverse myelitis, and also, in general, on the implications of the bodies of the analyst and the patient in the transference countertransference relationship. The patient started psychoanalysis while grieving over the death of her newborn son two months earlier. The analytical process took place while she rehabilitated from a postpartum transverse myelitis, and treated a Sjögren's syndrome. Ms. A mourned and elaborated conflicts during the years of analysis, finding a more depressive and active position, as well as a more reparative and less masochistic attitude. She managed a comfortable and satisfactory life with her own body, and its new realities. Pregenital regressions, such as masochism, are defenses against a narcissistic wound, adversity, as well as genital and depressive anxieties. The analytic attitude is irreplaceable to allow the patient to move on, integrate, and symbolize. Especially when transference is so demanding, and primitive, with manifest contents so tragic and painful, always challenging neutrality and abstinence. The body is part of the associative chain and transferential interpretations are useful, after all, the self is bodily.
