This paper seeks to explore how bodily experiences can often express encapsulated and therefore unmentalized trauma in primitive psychopathology. The author attempts to show specifically herein how obesity, seen in the patient presented, functions as a fatty capsule or encasement used defensively in a self-sensuous, autistic manoeuver to attenuate what has been referred to in the literature as “black hole” anxieties or “primitive agonies” as termed by Winnicott, which must be encapsulated. Furthermore, the author strives to consider the implications of gross deficits in mental space for the patient in and out of analysis. The ramifications of this lack of space for the analyst who endeavors to promote and create capacities for self-reflection as a necessary condition for self-awareness and introspection is illustrated here through this unusually challenging and protracted analysis.
