This paper describes my psychoanalytic work with a female to male transsexual patient who presented with depression and intense feelings of loneliness. The aim here is to search for a psychoanalytic, theoretical and clinical point of view, as well as providing a clear description of the setting. The work lasted a year. While it's true that twelve months of analysis may not be sufficient to observe the process in progress, it is equally true that circumstantial clinical reports concerning adult transsexual patients in psychoanalytic treatment are very rare. In that regard this paper hopes to contribute in accumulating psychoanalytic understanding with this group of patients. Constant attention was given to the transferential relationship of the analytic couple and the continuous use of the countertransference on the part of the analyst. The work puts us in touch with the limit of our instrument and with a ‘bedrock’ that frustrates analytic thinking from the start. The patient could not tolerate thinking and revealed a very powerful resistance: a tendency to attribute every unhappiness and symptom to the problem of gender. The ‘misfortune’ of being born in a ‘wrong’ body is ‘used’ internally as a resistance to thinking.
