This response expresses appreciation for the quality of Dr. Hershberg's discussion as it both affirmed my contribution and offered a supplementary perspective. Discussing my case in intersubjective systems terms, she helped me to better appreciate its interactive complexity and to rethink how the patient and I co-constructed this period of clinical momentum as well as the previous period of clinical stasis. Applying her perspective led me to identify the implicit rules that seemed to govern the mutual regulatory system that David and I evolved and to reconsider the multiple effects of my empathic stance. In turn, this reconsideration prompted me to raise questions about the best treatment approach to addictive problems, particularly how therapists may unwittingly reinforce or enable their patients' addictive tendencies.
