When an adult patient has an “actualized” unconscious fantasy, the analyst's interpretation alone is not enough to complete a working-through process that will resolve the fantasy's influence on the patient; a modification of therapeutic technique is required. In this paper, we first define what we mean by “actualized” unconscious fantasy and describe the “therapeutic play” we see as the most effective treatment for this condition. We then turn to a detailed account of one woman's treatment process to illustrate the modified technique and the theory behind it.
