The author briefly outlines the still unfinished analysis of a man of forty whose presenting symptoms were general feelings of unhappiness and the belief that he was shunned because it was visible to others that he was feminine and a masturbator. He also suffered from fear of homosexuality and sexual masochism, occasionally connected with fetishistic practices and sadistic fantasies. His family constellation consisted of an ineffectual, wife-dominated father, the reality of whose existence the patient denied, and an extremely domineering mother whom he hated but whose domination he could not evade. The wish to kill the mother and to have homosexual relations with the father clearly appeared in the course of analysis.
The usual paranoid mechanism involving the projection of the patient's aggression onto the father was displaced onto the mother; she, therefore, became the persecutor and became identified with the stools. The analysis revealed a massive fixation at the anal sadistic level with passive submission to the mother's sadistic impulses and a resulting intolerable narcissistic injury which was in turn repaired by the mechanism of introjection of the phallus according to common homosexual mechanisms.
