Observation of a man who experienced episodes of wishing to change sex, acted upon to the point of cutting his penis, is reported. The patient was terrified of losing his relationship with his mother if he became at all independent, causing a wish to give up his masculinity and fuse with her. The inevitable oscillations involved in such a megalomanic hope indicate that surgery would be a futile way to try to resolve the patient's difficulty.
