Flournoy claims that there are three levels of satisfaction involved in orgasm: narcissistic, pregenital, and oedipal. These three levels are also involved in the report of orgasm in the analytic situation. Flournoy believes that an impasse results, preventing the possibility of speaking successfully about orgasm in analysis. The analysis of a successful orgasm would be seen as a betrayal of the coupling in which the orgasm occurred, while the orgasm itself is seen as the betrayal of the analyst in the love for another. Just as orgasm excludes the analyst, the sharing of the report of the orgasm excludes the partner. Hence it is exceptional that a successful orgasm is reported in analysis. It is usually passed over with a comment, "It was good." Flournoy considers a case history in which a patient seemed to separate his orgasm from the actual coital ejaculation. This "after the fact" orgasm may possibly have derived from his wish to save the orgasm for the analyst.
