Current paradigms frame addiction primarily as a behavioral disorder, leading to treatment approaches focused on behavior modification and success measured solely by abstinence. This paper challenges this limited perspective by drawing on neuroscience and psychoanalysis to propose a novel framework that prioritizes achieving a fulfilling life as the ultimate goal, utilizing examination of subjective experience as the pathway. Neuroscience findings on reward/anti-reward dysregulation suggest three distinct brain configurations corresponding to three fundamental modes of living: a fulfilling life, a neurotic life, and an addictive life. While individuals naturally navigate between these states, one typically dominates. This paper redefines neurotic life, not as Freudian id-ego conflicts, but rather as the incessant pursuit of safety and certainty driven by feelings of lack, yearning, and fear that our society often perceives as normative. Furthermore, it argues that the behavior-centric approach can contribute to oscillations between addiction and neurosis commonly known as the “revolving door” of addiction. In contrast, the traditional psychoanalytic approach offers a more holistic path toward a fulfilling life. Its in-depth understanding of human suffering, emphasis on subjective experience, and use of the therapeutic alliance make it a valuable tool for addressing the underlying dynamics of addiction. This framework shift encourages a move beyond mere behavior modification toward facilitating a meaningful and satisfying life, ultimately offering a more sustainable and impactful approach to addiction treatment.
