This discussion of Shelley J. Klein’s “The Breakdown Happened—and What I Could Not Hear” honors the clinical courage required to write honestly about a case that unraveled and uses her paper as an occasion to examine what psychoanalytic training has failed to prepare clinicians to hear. The author argues that the relational and intersubjective framework Klein employed, though genuinely valuable, contains a structural blind spot in the presence of emerging psychosis: it presupposes a capacity for mutual subject recognition that psychosis is precisely dismantling. Drawing on the tradition of psychoanalytic work with psychosis and on the author’s clinical and theoretical work, the discussion identifies missed clinical opportunities—including engagement with substance use as a reality-level risk, closer investigation of the patient’s internal figure, “Kevin,” attention to the non-psychotic alliance, and the ongoing construction of a narrative linking breakdown to life history—and situates them within the broader institutional failure of psychoanalysis to train clinicians for work with psychosis.
