Treating an identical twin with an eating disorder poses special challenges highlighting separation-individuation and attachment issues. Where parenting is less than “good enough,” twins become their own dyad using each other for self-regulation at the price of impaired relationships with others. When that bond is threatened, the eating disorder serves as a substitute “evoked companion.” The paper draws on Stern's infant research and Bowlby's attachment theory.
